If your unit does not have access to DISS, submit a DOE Form 5631.20 to:
If your unit has the capability to submit your clearance through DISS, please submit your information into DISS, complete the Sandia Badge Request Form, and send to:

Badge recipient(s) must provide proof of identity at the time of badge pick-up. If a driver’s license (DL) or state-issued identification (ID) card is used for this purpose, it must be from a state/territory compliant with the REAL ID Act of 2005. Non-compliant DLs or IDs will NOT be accepted; an alternate photo ID will be necessary. Refer here for more information, including the most current list of non-compliant states/territories.

The following items are prohibited on Sandia premises inside Tech Area 1: Any electronic devices, including but not limited to, cell phones (e.g., BlackBerry, iPhone, Android, etc.), laptop computers, removable computer media (e.g., CDs, USB Drives, etc.), audio and visual recording devices, portable and wireless devices with transmitting capability, 2-way pagers, Apple devices (e.g., iPod, iPad, Smart watches etc.), e-readers (e.g., Kindle, Nook, etc.), and Bluetooth. Intoxicants, illegal drugs, drug paraphernalia, firearms, and explosive materials are also prohibited.
