What you need to know

Prepare for Your Visit

When you visit an on-site clinic or receive virtual care, be ready to provide:

  • Your insurance card (if you’re covered under a Sandia health plan)
  • Your health status and medical history
  • Your medications
  • Your allergies
  • Contact information for your primary care physician and any specialists you see

Using the FollowMyHealth Portal

In addition to scheduling appointments through the FollowMyHealth portal, you can use the portal for:

  • Secure, 24/7 access to your medical records
  • Prescription renewal requests
  • Secure email exchanges with your on-site doctor

It’s easy to access the portal. To get started, follow these steps:

  1. Request a FollowMyHealth registration code.
  2. When you receive your code or if you’re a returning visitor, go to the login page and follow the prompts to log in.

After you’ve set up your account, you can download the FollowMyHealth portal app from the App Store or Google Play.

Complete your health assessment

Completing an annual health assessment enables you to evaluate your current health status, track progress and identify any potential setbacks.

By pairing the health assessment with health coaching, you create a powerful strategy for heightened awareness and actionable steps that lead to improved well-being.
