This corporate procedure applies to all Sandia National Laboratories (SNL) organizations, all management elements, all SNL sites, and all Members of the Workforce.

Granting or permitting exceptions or violations of policy, process, or procedure without authority, regardless of position or title, may be cause for disciplinary action up to and including termination of employment. This procedure describes the requirements for Members of the Workforce to follow while using government-owned or -controlled vehicles (rental and privately owned vehicles (POVs) used for government purposes) and for reporting vehicle accidents.

- The Traffic Safety contact in the Safety Engineering Program.
- DOE, if it meets the occurrence reporting criteria (see ESH100.4.RPT.3, Report Occurrences)
- Determine whether an accident is a reportable occurrence, investigate the accident, and identify causes and corrective actions.
- Notify the California Department of Motor Vehicles (DMV) when any of the following results from a vehicle accident:
- Vehicle damage exceeds $750.
- Injury, regardless of severity.
- Death.

- Inform responsible managers of any accidents that have resulted in personal injury or damage to a vehicle that was approved for use and was in use during the conduct of Sandia business.
- Report the accident within 10 days to the California DMV at SNL/CA, whether or not you caused the accident, and even if the accident occurred on private property. Note: See the CA Driver Handbook, "Reporting the Accident to DMV," for additional information.
- Follow the instructions in Table 1.

- Report the accident to the Sandia Fleet Management at (505) 284-2543 within 1 workday of the accident.
- Complete GSA Standard Form 91, Motor Vehicle Accident Report, (located in the glove box of each vehicle). Complete the form within 5 working days and fax it to GSA-AMC at 1-816-823-3634. Submit the original copy to the Traffic Safety contact in the Safety Engineering Program.
- Arrange for a vehicle damage estimate(s).For damages less than $2,500, at least one estimate is required. For damages greater than $2,500, at least three estimates are required. Contact the GSA AMC for locations of estimate providers.
- Fax a copy of the police report to the GSA-AMC at 1-816-823-3634 for offsite accidents only.
- Report to a Sandia health services facility prior to returning to work to initiate an SF 2050-P if you are a Sandia employee who was injured in an accident while on official travel. Failure to do so may jeopardize worker compensation benefits.
- Retain copies of any documents created during the initial and subsequent investigations.
- Notify the Department of Transportation (DOT) at 1-800-424-8802 immediately, and complete DOT Form F5800.1, Hazardous Material Incident Reporting, if the accident involved vehicles transporting hazardous material.

- Notify the rental company and complete any accident forms required by that company. The forms are usually located in the vehicle glove box.
- Notify US Bank (corporate VISA card company) at 1-800-VISA-911 within 20 days of the date of the accident for any car that was rented from a company other than Avis.
- Notify the Pension Fund & Savings Plan contact of a claim or reportable event, including damage to non-Sandia owned property by a government owned vehicle, rental vehicle or other device. See the Sandia Travel Site or call the Travel Help Line at (505) 844-8000 for specific information on insurance coverage limitations related to rental cars. Also, see FIN100.1.TNT.5, Travel on Business. Note: If the driver abuses the rental vehicle, drives recklessly, or drives under the influence of alcohol or drugs, the driver is liable for the full amount of any loss or damage.
- Notify the proper Sandia contact.
- SNL/NM: the Traffic Safety contact in the Safety Engineering Program.
- SNL/CA: Herman Armijo.
- TTR: the site ES&H Coordinator.
- KTF: the resident Range Manager.
- Retain copies of the police report, rental agreement, accident report, and insurance documentation for the vehicle rental agency.
- Report to a Sandia health services facility prior to returning to work to initiate an SF 2050-P, Report of Occupational Injury/Illness, if you are a Sandia employee who was injured in an accident while on official travel. Failure to do so may jeopardize worker compensation benefits.

- Report vehicle accidents to the Federal Motor Carrier Safety Regulations (FMCSR) Administrator. A copy of GSA Standard Form 91, Motor Vehicle Accident Report, is sufficient.
- Report to a Sandia health services facility prior to returning to work to initiate an SF 2050-P, Report of Occupational Injury/Illness, if you are a Sandia employee who was injured in an accident while on official travel. Failure to do so may jeopardize worker compensation benefits.
- Avail yourself for testing for alcohol or controlled substances within 8 hours and 32 hours, respectively, following an occurrence involving a commercial motor vehicle operating on a public road for business purposes, or as soon as practical. A post-accident test is required to be conducted, if a citation was issued to the CMV driver and the accident involved one or more of the following:
- Human fatality.
- Bodily injury with immediate medical treatment away from the scene.
- Disabling damage to any motor vehicle or to any motor vehicle requiring tow away.
