Applicability, Exceptions, and Consequences

This corporate procedure applies to all Members of the Workforce who identify, submit, or use lessons learned.

Procedure Overview

This procedure describes the requirements for identifying, submitting, and using lessons learned through the SNL Lessons Learned system. 

SNL's ES&H Operating Experience or Lessons Learned Program helps implement SNL's Integrated Safety Management System (ISMS) Feedback and Improve safety management core function. The program captures, analyzes, and disseminates ES&H lessons learned information which applies to line operations. The goal of SNL’s ES&H Lessons Learned Program is to assist all personnel in the safe and efficient conduct of work by learning from the past experiences and avoiding the recurrence of unplanned events that adversely impact personnel, the environment, or mission results.

Procedure Requirements

Coordinate the Lessons Learned Program ES&H Coordinators
  • Assist line organizations in implementing the ES&H Lessons Learned Program.
  • Submit, as appropriate, ES&H lessons learned to the Lessons Learned Program via the template provided at the ES&H Lessons Learned Program website.
  • Identify whether lesson learned should be submitted to a targeted audience.  If so, indicate the target audience when completing the lesson learned template provided at the ES&H Lessons Learned Program website.
  • Assist the Lessons Learned Program Manager with the identification and dissemination activities of the Lessons Learned Program.
  • Ensure that lessons learned documents are reviewed for security classification and control.
  • Ensure that submitted lessons learned have received all applicable classification reviews prior to dissemination.
  • Identify opportunities for improvement for the ES&H Lessons Learned Program website.
Identify and Process Lessons Learned Managers
  • Identify and submit lessons learned in accordance with the requirements of ESH100.4.RPT.3, Report Occurrences, to the occurrence reporting (OR) contact.

    Note: See the occurrence management lessons learned website for information and assistance with creating and submitting lessons learned in occurrence reports.

  • Identify whether lesson learned should be submitted to a targeted audience.  If so, indicate the target audience when completing the lesson learned template provided at the ES&H Lessons Learned Program website.
  • Include in organization policies and procedures identification of lessons learned when activities involve identifying and correcting problems, diagnosing issues, or implementing corrective actions.
  • Incorporate lessons learned into operations, training, maintenance and work planning, work processes, design, and construction.
  • Collect and disseminate lessons learned information from sources such as organization-specific occurrences and operations information obtained from similar DOE facilities. See ES&H Lessons Learned Program website.
  • Review applicable lessons learned information and apply the information to the organization’s operations.
  • Disseminate lessons learned information to Members of the Workforce and other organizations that are performing similar work.
  • Take actions to minimize or prevent recurrence.
Use Lessons Learned Members of the Workforce
  • Submit, when appropriate, ES&H lessons learned to the Lessons Learned Program via the template provided at the ES&H Lessons Learned Program website.
  • Identify whether lesson learned should be submitted to a targeted audience.  If so, indicate the target audience when completing the lesson learned template provided at the ES&H Lessons Learned Program website.
  • Use, as appropriate, the subscription service to automatically receive lessons learned information related to the specific hazards in their work environment.
  • Review and use lessons learned information from the ES&H Lessons Learned Program website and other sources (e.g., email from ES&H Communications) as an aid in planning and performing work safely.

    Note: Members of the workforce may also receive lessons learned that are communicated to them through targeted TEDS classes.

  • Consult the lessons learned contact or the appropriate division ES&H team for additional information about the Lessons Learned Program.

  • Review external and internal lessons learned.
  • Determine if targeted audience should receive lessons learned.
  • Identify TEDS classes associated with targeted audience.
  • Send out targeted lessons learned using identified TEDS classes
  • Ensure that lessons learned has been sent out to targeted audience.
