Applicability, Exceptions, and Consequences

This corporate procedure applies to all Members of the Workforce who operate or supervise those who operate cranes or hoists.

Procedure Overview

This procedure lists the minimum requirements that must be followed for operations on SNL-controlled premises involving overhead cranes, mobile cranes, and hoists. Manufacturers' requirements must be followed if they are more stringent than requirements discussed in either DOE-STD-1090-2007 or this procedure. For questions regarding hoisting and rigging, consult the cranes, hoists, and rigging program contact or the Material Handling ES&H Safety Engineering website. For terms and definitions related to material handling see DOE-STD-1090-2007, Chapter 1, "Terminology and Definitions."

Procedure Requirements

Training

Qualifications

  • Ensure that only qualified SNL operators use SNL-owned, rented, or leased material-handling equipment.
  • Ensure that your organization’s operators/rigging inspectors(QREIs) maintain their skills and qualifications by implementing the following:
    • Ensure that trained operators and inspectors receive familiarization training (on-the-job [OJT]) with the line-supplied material-handling equipment.
    • Ensure that operators or inspectors use or inspect material-handling equipment frequently enough to acquire skills and maintain them. Note: It is recommended to have operators use equipment at least 1 hour per month and inspectors perform 2 hours of inspections at least every 6 months. 
    • Maintain appropriate qualification forms (see ESH100.2.IS.21 ) in operator’s organizational files).

(The requirement for the use of the material-handling qualification forms started on October 12, 2009, and is not retroactive. New operators and inspectors will be given the qualification forms upon successful completion of applicable training courses. Currently, qualified operators and inspectors will be given these forms after every 3-year refresher training cycle.)

  • Fulfill DOE-STD-1090-2007, Chapter 6, "Personnel Qualifications and Training."
  • Complete applicable SNL-specific training (see ESH100.2.IS.21 ).
Mobile crane operators (MCO) SNL/CA Obtain a valid certificate of competency issued in accordance with 8 CCR §5006.1 by an accredited certifying entity for the type of crane to be used.
Mobile crane operator (MCO) SNL/NM
  • Obtain and maintain a crane operator's license from the State of New Mexico. Note: SNL is using an alternative qualification program (approved by the state) provided by the National Commission for the Certification of Crane Operators (NCCCO) for SNL employees who operate mobile cranes of 2 or more tons in capacity and who cannot qualify under the New Mexico Hoisting Operators Safety Act (HOSA) requirements. Consult the material-handling SME for additional information.
  • Pass an annual Department of Transportation (DOT) drug screen and physical exam (administered by SNL medical).

Note: Operators are then part of the Mobile Crane Operators Drug Screen Program (random testing).

Inspections and Maintenance

Determine Inspection and Maintenance Frequency SNL inspectors and maintenance personnel

Use the following for determining inspection and maintenance frequency:

  • Check manufacturer requirements for inspection and maintenance (including any special lubrication).
  • Is the maintenance frequency adequate for the use of the lifting device (i.e., number of lifts per hour/day and percent of lifts near capacity) (see Material Handling ES&H Safety Engineering website).
  • Environmental exposures (windy, dusty, raining, hot or cold, radiation, chemical, etc.)
  • Extent of conditions.

Note: It is recommended to periodically analyze and review maintenance records (oil analysis, trouble calls, and manufacturer’s requirements).

Request or Perform Inspection and Maintenance Equipment owners or operators

Request or perform inspections and maintenance as necessary for:

  • Initial and periodic/annual inspections, load tests, repairs, modifications, and preventive maintenance. (SNL line organizations can get inspections and maintenance by calling Facilities Management and Operations Center at 844-4571 and filling out SA 6610-FES, Request for Site Services [Word file/Acrobat file].)
  • Rigging equipment inspection (see QREI list for qualified inspectors). Note: A qualified rigging equipment inspector (QREI) inspects rigging equipment annually or as required.

Critical Lifts

Ensure that an appointed person classifies each lift into one of the DOE categories (ordinary, critical, or pre-engineered production) prior to planning the lift. (See DOE-STD-1090-2007 for the definition of critical lifts.)

In addition to the DOE-STD-1090-2007 requirements, ensure that:

  • Rigging devices used in critical lifts must have been proof tested or have evidence of proof testing, such as the manufacturer’s proof-testing certificate.
  • A critical lift plan is prepared in accordance with the Critical Lift Plan on the Material Handling ES&H Safety Engineering website.

Note: Some chapters in the DOE-STD-1090-2007 have chapter-specific critical lift requirements. Check each chapter for special critical lift requirements.

Comply with Requirements for Critical Lifts Members of the Workforce
  • Follow the requirements in DOE-STD-1090-2007, Chapter 2, "Critical Lifts," and Critical Lift Plan on the Material Handling ES&H Safety Engineering website.
  • Complete an SF 2001-CLD, Critical Lift Data Sheet (Word file/Acrobat file) for the planned critical lift.

Lifting Personnel

Hoists

Notes:

  • Chapter 8 applies to the following types of equipment:
    • Overhead hoists (underhung).
    • Jib cranes/hoists (floor- and wall-mounted).
    • Manual-lever-operated hoists (wire rope, chain, and web-strap types).
  • These lifting devices require initial, pre-use, and annual inspections. See the inspection summaries on the Material Handling ES&H Safety Engineering website. (Frequent (monthly) inspections of manual-lever hoists do not require documentation.) .

Mobile Cranes

  • Follow the requirements in DOE-STD-1090-2007, Chapter 9, which states that mobile crane operators in New Mexico who operate cranes or hoists with a capacity of 2 tons or more must comply with Hoisting Operators Safety Act (HOSA). (See the ESH100.2.IS.21 for more information).

Note:  Consult the wind advisory contact. Wind speeds may be reported in knots, which can be multiplied by 1.15 to obtain mph equivalent.

Use Mobile/Tower Cranes that may exceed 200 feet in height Managers and Members of the Workforce

Notify the administrator of the FAA regarding any of the following construction or alterations (as detailed in CFR Title 14 Part 77.13):

  • Any construction or alteration exceeding 200 ft above ground level.
  • Any construction or alteration:
    • Within 20,000 ft of a public or military airport which exceeds a 100:1 surface from any point on the runway of each airport with at least one runway more than 3,200 ft.
    • Within 10,000 ft of a public or military airport which exceeds a 50:1 surface from any point on the runway of each airport with its longest runway no more than 3,200 ft.
    • Within 5,000 ft of a public use heliport which exceeds a 25:1 surface.
  • Any highway, railroad or other traverse way with a prescribed adjusted height exceeding the above noted standards.
  • When requested by the FAA.
  • Any construction or alteration located on a public airport or heliport regardless of height or location.
  • Obtain Federal Aviation Administration (FAA) approval using FAA Form 7460-1, Notice of Proposed Construction or Alteration. This form must be submitted to the FAA a minimum of 30 days before using a crane.
  • Contact the Albuquerque FAA office.
  • Notify the Albuquerque International Sunport.
  • Contact SNL Aviation Safety.

Note: Managers and Members of the Workforce should consult the cranes, hoists, and rigging program contact for assistance.

Wire Ropes and Slings

  • A QREI inspects all rigging equipment annually.
  • SNL slings have a permanent means of identification (e.g., an attached tag or label) with a unique stenciled or stamped SNL identifier. The identification numbers are organization specific and must have a highly visible due date for the annual inspection.

Rigging Accessories

Ensure that:

  • Rigging accessories (that require documentation) have a permanent means of identification (e.g., an attached tag or label) with a unique stenciled or stamped SNL identifier. The identification numbers are organization-specific and must have a highly visible due date for the annual inspection.
  • Inspection of all new and repaired hooks prior to initial use by a QREI:
    • Dimensional data on the hooks must be recorded to facilitate subsequent inspections for wear and throat openings.
    • Dated and signed inspection records must be kept on file and must be readily available.
  • For “other” rigging equipment, an annual inspection, which may require disassembly, of shackles, eyebolts, turnbuckles, links, rings, metal plate clamps, swivels, swivel hoist rings, precision load petitioners, load-indicating devices, and compression hardware is performed by a QREI. Periodic inspection intervals can not exceed one year (for additional information, see the Material Handling ES&H Safety Engineering website). (Written inspection records are not required.)

  • The identification numbers are organization-specific and have a highly visible means of noting the annual inspection due date.
  • Note: Members of the Workforce who design new, or modifications to, special-use lifting devices should see SNL DG 10221/A and SNL Design Guides 10220 and 10220 for guidance for designing, approving, fabricating, and testing these devices. MOWs operating cranes and hoists, handling rigging, or working around any material handling equipment shall work with caution, which includes:

  • NOT working under or near suspended loads.
  • Keeping fingers away from pinch points.
  • Using tag lines as much as possible.
  • Understanding that you are responsible for the lift, and that anyone can stop a lift, if a hazardous situation is observed.
  • Wearing proper PPE (the hazard of the lift will determine the level of protection needed).
