Applicability, Exceptions, and Consequences

This procedure applies to all Members of the Workforce who participate in activities involving confined spaces and telecommunication field installations at all Sandia-controlled premises. 

All sites follow SNL/NM requirements unless specified otherwise in this procedure.  A site (e.g., Tonopah Test Range, or Pantex) may use equivalent methods, services, and materials, if pre-approved by the Confined Space Subject Matter Expert.

This procedure does not apply to:

  • Excavations, such as trenches.
  • Underground construction, such as a tunnel, shaft, chamber, and/or passageway.

Exceptions to, or deviations from this procedure must be approved through the Executive Policy Sponsor or Policy Area Manager, if delegated. Granting or permitting exceptions or violations of policy, process, or procedure without authority, regardless of position or title, may be cause for disciplinary action up to and including termination of employment. Procedure Overview

This procedure describes requirements for identifying, classifying, and entering both non-permit and permit-required confined spaces.

Identifying and Classifying a Confined Space

  • Determine whether it is a non-permit confined space or permit-required confined space, and

3 Confined space owners Incorporate requirements for permit entry—including C5 – Alternate Procedures and/or C7 - Reclassification entry options— into applicable work control documents, if the space owner or his designee enters and conducts work inside permit-required confined spaces.

Interpret the Absence of Signage Members of the Workforce
  • Regard signs as the principal method of warning.
  • Do not interpret the absence of signage as an indication that an area is not a confined space if the area otherwise appears to qualify as a confined space.
  • Consult the division ES&H team industrial hygienist for an evaluation of the area, if unsure whether a space is a confined space.

Obtaining a Permit for Entry to Permit-Required Confined Spaces

Contact the confined space owner to identify the following:

  • Current status of permit-required confined spaces (e.g., decommissioned steam pit may no longer be a permit-required confined space).
  • Modifications/alterations to permit-required confined spaces (e.g., a space retrofitted with nitrogen gas lines may add a new hazardous condition to mitigate or eliminate).
  • Conditions or activities that may adversely impact permit-required confined space entry (e.g., impairment of egress due to elimination or sealing of an access portal).

Obtain an Entry Permit 

 

SNL/NM Member of the Workforce
  • Contact the division ES&H team industrial hygienist  prior to the initiation of entry operations, when a hazard may be introduced to the space that was not previously evaluated during the initial space classification. Examples of such hazards include:
    • Hot work, including welding, cutting and/or brazing.
    • Use of hazardous chemicals (including gases).
  • Obtain SF 2001-CSE, Confined Space Entry Permit by initiating the on-line permit issuance process.
  • Use SF 2001-CSS, Confined Space Permit Sign-In/Sign-Out Sheet for Emergency Response when additional entrants are identified after the online portion of the permit has been completed.

  • Contact the division ES&H team industrial hygienist to evaluate hazards and determine safety requirements for each confined space entry.
  • Obtain SC 2001-CSE, Confined Space Entry Permit by accessing this link.

Preparing for Entry into Permit-Required Confined Spaces

Step Responsible Individual Required Action
1 Supervisor authorizing entry (SAE) Ensure entrants or attendants are current with required training.
2 SAE

Conduct a pre-job briefing with all entrants and attendants to review:

  • Work to be performed and any prohibited activities.
  • Anticipated hazards, including signs and symptoms of exposure to the hazards.
  • MSDSs, attached to the permit, for hazardous chemicals to be used in the space.
  • Hazard control measure.
  • Rescue and emergency notification procedures.
3 SAE Complete the required fields on the permit in the required sequence.
4 SAE Establish a means (e.g., telephone, cellular phone, or two-way radio) at the entry site to summon rescuers.
5 SAE

Obtain the appropriate retrieval apparatus (or system):

  • At SNL/NM, contact the confined space entry/equipment issuance contact.
  • At SNL/CA, contact the division ES&H team industrial hygienist.
6 SAE
  • Conduct pre-entry functional (bump) test according to the instrument manufacturer’s guidance.
  • Replace instruments that can not be properly calibrated or when the annual factory calibration has expired.
7 SAE

Conduct pre-entry atmospheric testing:

  • Test for oxygen content, combustible concentrations, and concentrations of other potential atmospheric hazards, as anticipated, in that order.
  • Test at all levels and locations within a space where entrants may be exposed.
  • Ensure measurements are within acceptable entry reference levels prior to entry and for the duration of the entry.
  • Replace instrument if it malfunctions or gives erratic readings.
8 SAE

Ensure necessary procedures, practices, and equipment for safe entry and any non-entry rescue (e.g., retrieval apparatus (or system) are in place.

  • Implement positive ventilation at all times during an entry when a potential or actual hazardous atmosphere exists.
  • Ventilate the space until the atmosphere is within acceptable entry reference levels (AERLs) before entry and for the duration of entry.
9 SAE

Contact the following prior to entry and suspend entry if advised that rescue services are unavailable:

  • At SNL/NM: SNL EOC to confirm availability of rescue services by calling 844-4189
  • At SNL/CA: Central Alarm Station (CAS) extension 4-2300, and requests notification of the Alameda County Fire Department Dispatcher (located at Lawrence Livermore National Laboratories [LLNL]) to confirm the availability of rescue services.
10 SAE

Authorize entry (by entrants listed on the permit) only if all the following are true:

  • Entry conditions are acceptable and all specified control measures are implemented.
  • Entry is only made during the day and time specified on the Permit,
  • Attendant is on duty and controls the entry by remaining at the permit-required confined space.

Entry into Permit-Required Confined Spaces

Prohibit Entry into a Permit-Confined Space Supervisor authorizing entry (SAE) Remove, or cause to be removed, unauthorized individuals who enter or attempt to enter a permit-required confined space during entry operations.
Supervise Entry into a Permit-Confined Space SAE

Verify that entry operations maintain acceptable entry conditions, including acceptable entry reference levels, during:

  • Entry and at all times entrants are in the space, and
  • Transfer of the SAE’s responsibilities to another trained Member of the Workforce.
Ensure Safety of Entrants Attendants
  • Maintain effective and continuous communication with the entrant(s).
  • Perform no other duties that may interfere with the ability to observe activities inside and outside of the space to ensure entrant(s) safety.
  • Warn unauthorized individuals to stay away from permit-required confined space, advise them to exit if they have entered, and inform SAE and entrants of any unauthorized individuals who have entered the permit-required confined space.

Emergency and Rescue

  • Evacuate space in the event of a prohibited condition (e.g., hazardous atmosphere) or the detection of entrant behavioral effects (e.g., loss of coordination) associated with a hazardous exposure.
  • Call SNL EOC, 844-0911 (at SNL/NM) or CAS, 4-2222 (at SNL/CA).
2 Entrants Exit space immediately (i.e., self-rescue) if capable. Communicate any egress impairment to Attendant. Wait for rescue instructions, if you are physically unable to evacuate without outside assistance.
3 SAE or Attendant or other authorized entrants outside of the space
  • Initiate a non-entry rescue (e.g., use of retrieval apparatus [or system]), if:
    • Non-entry retrieval equipment is in place;
    • Entrants are attached to retrieval lines;
    • Retrieval is possible without additional help and does not require entry by the attendant; and
    • Non-entry rescue is necessary and won’t injure or further injure entrant(s).
  • Wait for rescue and emergency services if unable to attempt or complete a non-entry rescue. Note: Do not enter space to perform rescue.

Terminate Entry and Cancel Permit

  • At completion of job, or end of work shift, whichever occurs first.
  • If new hazards are encountered that are not covered in the permit.

C5 Alternate Procedures for Entry into PRCSs

Ensure that:

  • A C5 – Alternate Procedure is used for entry into a permit-required confined space when the only hazard is a potential or actual hazardous atmosphere, which can be controlled through positive ventilation alone.
  • The division ES&H team industrial hygienist is contacted to determine whether a C5 alternate procedure entry is appropriate and to establish the acceptable entry conditions.
  • Work control procedures are written to implement the C5 alternate procedure entry conditions established by the division ES&H team industrial hygienist.
  • The proper form is used for all C5 alternative procedure entries:
    • At SNL/NM use SF 2001-CSA, Confined Space Certification - C5 Alternate Procedures.
    • At SNL/CA, use SC 2001-CSA, Confined Space Certification - C5 Alternate Procedures.
  • Members of the Workforce participating in C5 alternate procedure entries are properly trained:
    • At SNL/NM, CNF104.
    • At SNL/CA, CNF105 and CNF107.
Supervise C5 Alternate Procedure Entries SAE

Ensure that:

At SNL/NM:

  • Use work control procedures specific to the C5 alternate procedure entry and the work to be conducted in the space.
  • Follow, complete, and sign the SF 2001-CSA,Confined Space Certification - C5 Alternate Procedures form.
  • Post the C5 Alternate Procedures form for all entrants to read.
  • After the termination of the entry activity, submit the completed C5 Alternate Procedures form to the Customer-Funded Records Center.

At SNL/CA:

  • Contact the division ES&H team industrial hygienist to determine if the conditions for the specific entry qualify for the C5 Alternate Procedure.
  • Use work control procedures specific to the C5 alternate procedure entry and the work to be conducted in the space.
  • Follow, complete, and sign the SC 2001-CSA, Confined Space Certification - C5 Alternate Procedures form.
  • Post the C5 alternate procedure form for all entrants to read.
  • After termination of the entry activity, submit the completed C5 Alternate Procedures form to the Industrial Hygiene Group, MS 9956.

C7 Reclassification for Entry into PRCSs

Manage C7 Reclassification Entries Managers

Ensure that:

  • A C7 – Reclassification procedure is used for entry into a permit-required confined space as a non-permit confined space, when there is no potential or actual hazardous atmosphere and all remaining potential hazards may be eliminated without entry into the space.
  • The division ES&H team industrial hygienist is contacted to determine whether a C7 reclassification entry is appropriate and to establish the acceptable entry conditions.
  • Work control procedures are written to implement the C7 reclassification entry conditions established by the division ES&H team industrial hygienist. The proper form is used for all C7 reclassification entries:
    • At SNL/NM, use SF 2001-CSR,Confined Space Certification C7 – Reclassification.
    • At SNL/CA, use SC 2001-CSR, Confined Space Certification C7 – Reclassification.
  • Members of the Workforce participating in C7 reclassification entries are properly trained:
    • At SNL/NM, CNF050.
    • At SNL/CA, CNF105 and CNF107.
Supervise C7 Reclassification Entries SAE

Ensure that:

At SNL/NM:

  • Use work control procedures specific to the C7 reclassified space entry and the work to be conducted in the space.
  • Follow, complete, and sign the SF 2001-CSR,Confined Space Certification – C7 Reclassification form.

At SNL/CA:

  • Contact the division ES&H team industrial hygienist  to determine if the conditions for the specific entry qualify for the C7 Reclassification.

Entry into Non-Permit Confined Spaces

Prepare for Entry into a Non-Permit Confined Space (NPCS) Managers

Ensure that Members of the Workforce who plan to enter a non-permit confined space do the following prior to entry:

  • Complete the required training for non-permit confined space entry, CNF050, unless current with CNF104 or CNF105.
  • Contact:
    • At SNL/NM, the confined space owner to identify significant modifications or alterations to the space that could impact the classification (e.g., permit or non-permit) and possibly introduce new hazards.
    • At SNL/CA, the division ES&H team industrial hygienist  to complete an evaluation of the planned entry and to obtain a Non-Permit Confined Space Checklist.
  • Evaluate:
    • Changes in the confined space since it was designated as an NPCS, or if it has been closed up for a long time (e.g., oxygen deficiency due to rusting of interior).
    • Work being performed in the area (e.g., use of combustion engines that generate carbon monoxide) that could change relevant entry procedures or the classification of the space (e.g., permit or non-permit), or otherwise adversely affect entry.
    • Work performed and materials used within the space to determine any adverse affect on entry that could result in permit-required conditions (e.g., hot work or use of hazardous chemicals).
  • Contact the division ES&H team industrial hygienist for a re-evaluation of the space prior to entry, if permit-required conditions may exist.
  • Determine the necessity for emergency and rescue procedures for remote areas including:
    • Retrieval apparatus (or system).
    • Effective means to summon responders.
    • Radio check-in procedure, with a check-in frequency interval.

Contractor Confined Space Entry Activities: Construction and Services

  • Contracted work involving confined space entries meet the requirements of ESH100.1.GP.1, Manage Safety for Contracted Activities.

  • The confined space owner is contacted concerning the following prior to conducting confined space entries:

  • Hazards are identified and controlled within the permit-required space, and any necessary precautions or procedures to protect Members of the Workforce in or near the space are implemented.
  • Contractors provide a debriefing regarding any hazards confronted or created in permit-required spaces during entry operations to the SDR.
  • The division ES&H team industrial hygienist is provided debriefing information when unforeseeable hazards are confronted or created during a permit-required space entry.

Contractor Confined Space Entry Activities: Telecommunications

    • Contractors provide a debriefing regarding any hazards confronted or created in the spaces during entry operations to the SDR.
    • The division ES&H team industrial hygienist is provided debriefing information when unforeseeable hazards are confronted or created during a permit-required space entry.
