INITIAL CLEARANCE REQUEST

Initial Clearance Request – A request for individual applying for an initial DOE security clearance or has held a DOE security clearance in the past.

Prior to completing paperwork, please ensure that the SNL Manager or delegate has created an active relationship for the individual requiring a security clearance in Enterprise Person.

Required Forms Authorization Submittal

PERSEC 5633.33 - NNSA Personnel Security Clearance Action Request

  • On this form, select "Applicant" as the available clearance action.

A member of SNL management is required to complete:

  • Clearance Justification Narrative to justify the DOE clearance.
  • Supervisor Certification fields
  • Sandia Personnel Security requires a minimum of 5 working days to process a request.
  • All four required forms must be submitted together.

Clearance paperwork becomes Official Use Only (OUO) information when completed. OUO information must be encrypted with Entrust when transmitted over a network (including e-mail). Fax completed clearance paperwork if you do not have Entrust.

 

SF 7643-CEC- Contractor/Consultant Badge/Clearance Request Form

  • Request may not exceed the contract expiration date.
  • In Section 3, Badge Request, select "Clearance in Process (issue uncleared badge)".

The SNL Requestor must be an SNL Employee.

A member of SNL management is required to complete Section 5, Approving SNL Manager.

SF 2730-CIV- Contractor Pre-Processing Background Review Certification A company official must certify that the pre-processing background review has been completed and they feel the individual is a good candidate for a DOE security clearance.
DOE F 5631.18, Security Acknowledgement Form The individual must read, sign and return the form to the Personnel Security Clearance Office.

How will I know the badge is ready to be picked up?
An automatic e-mail notification from Personnel Security will be distributed to the SNL Requester listed on SF 7643-CEC, Contractor/Consultant Badge/Clearance Request Form. The SNL Requester can notify the contractor to report to the Badge Office to pick up their badge upon receipt of the automatic e-mail notification.

Contractors who do not require clearances and need an uncleared badge, are required to submit form SF 2730-UBR, Uncleared Contractor Badge Request Form.

How will my contractor receive clearance instructions and paperwork?
When the above forms are submitted, Personnel Security will distribute an e-mail to the clearance applicant with additional paperwork and instructions.
